Exemestane leads to bone loss

The aromatase inhibitor exemestane, which can prevent breast cancer in postmenopausal women, worsens age-related bone loss despite calcium and vitamin D supplements, a study finds. In a randomised trial around 350 women were given either exemestane or placebo plus calcium and vitamin D supplements for two years. Researchers found reductions in bone-mineral density (BMD) was over three times greater among exemestane recipients than with the placebo group. Density at the distal tibia and cortical thickness at the distal radius were similarly affected. The authors speculated the reduction was a secondary effect due to loss of estrogen. “Women considering exemestane for the primary prevention of breast cancer should weigh their individual risks and benefits,” the authors concluded.
